Registration forms and Supplemental forms

Below are all of the forms that are required in order to enroll as a new family at Foundations For Learning. The required forms are:

*New Enrollment Form
*Allergy and Food Preference
*Discipline Policy
*Enrollment Packet
*Tuition Agreement
*Sick Policy

In addition to all of the above forms, each child must have a current physical with immunization records as well as forms for all topical and non topical medications.

*Health form
-This form must be completely filled out before submitting it.

*Non-Prescription Topical Medications
-This form is used for: sunscreen, bug spray, hand lotion, chap stick, diaper cream, Vaseline, etc…

*Medication Administration Form
-This form is used for all prescriptions as well as any over the counter oral medications: Tylenol, benedryl, inhalers, medicated diaper cream, Epi-Pens, Neosporin, etc…

If your child has a known allergy, we will need to have a action plan on file. This form must be completed by both the doctor and the parents.
